Internal cooling missing on a lot of bigger high end amps?

A fan that is added to a design without cooling fins inside while using a fan is doing so to cool a poorly designed amp.

If you look at the older PPI power series amps before being sold off, they had internal fins inside for the air to move over and help cool the amp.

I generally agree with you about the internal fan, as a general rule.

The only thing to note is when there is a cost saving with a tolerable outcome.  Take my AA 5K vs DC 5K for example.

About a year ago, the AA cost $1200 while the DC cost $1800.  In both designs, the amps can be wired down low as shit (0.35 nominal), but the AA has 2 internal fans while the DC has none.  You look over the design, and you see that the 5K has all the output resistors grouped in the center, while the DC has them spread throughout the filtering side.  There may be other reasons for the fans, too.  Many of the resistors in that size range dont have a sink, and dont need them as long as they have a little airflow across them.  So if both amps perform the same or similar, the design may not be optimum, but it may still be acceptable.
