Mustang06331
There is a weight on all 5 speed fox bodies that eliviated this vibe that is inherent from ford . If your sure all your parts are good then goto the aluminum Motorsport d shaft, pricey but will cure about 75 % of the driveline vibe
Skullz
Best money spent was on the alum drive shaft on my 84 stang. EDIT: Poly bushings in front and back made it even better. 01 Ford focus ZX3 Pioneer AVH-X491BHS PPI PC 4800.2 Morel Maximo 6.5" x2
Kenyel Dotts
drive shaft weights are usually hard to nock off. In the case that It did fall off you would see a noticeable clean spot. I would go with the throw bearing or the bearing that's in the shaft coming out the transmission. Did you use the set tool for your clutch and clutch plate? If you can get it up in the air try having some one put the car in to gear and listen for a clunk. If you can see the clutch and clutch plate try to see if it jumps or moves in or out.
