How the volume was set is what kills it. My 40.1's are set with the gains on MAX. Never hurt a sub or an amp. . . Gain setting is over rated. . .User control of the volume knob is the actual problem. . . BRian Current system: 1997 Blazer - (4) Customer Fi NEO subs with (8) American Bass Elite 2800.1s Previous systems: 2000 Suburban - (4) BTL 15's and (4) IA 40.1's = 157.7 dB at 37 Hz. 1992 Astro Van - (6) BTL 15's and (6) IA 40.1's = 159.7 dB at 43 Hz. You can have the gains at max and the HU on vol 1 or gains down and HU vol max and get the same power. They depend on each other. In this case, if he really had the volume set on 3-5 like he states, I doubt he was getting anywhere near full output of the amp, even if the gain was set to max. . .maybe that was a typo.
