The flat areas, do those first (inner and/or outer).

Just avoid placing the material on areas that have bracing, you won't see much, if any, difference.

It would and you would make yourself a little enclosure for your speakers.

However most door speakers are designed to be ran as IB, so air space isn't an issue and output doesn't really change much between using them in a sealed or unsealed configuration.

Again this pertains to most door speakers, not all.
