Similar to an explosion shockwave, that is more positive pressure than rarefraction because this is the point that a total vacuum is created. That sound level inside a car would go right through ear protection and probably deafen you. If you were still alive after it scrambled your insides. But the sound outside the car will probably be enough to damage hearing.
